Support Coordination

Our Support Coordination services help participants and supporters to build capacity in managing their NDIS matters and obtain a clearer understanding of their schedule. In doing so, we provide the following services through our general Support Coordination:

Plan Implementation

Assisting participants to understand their NDIS plans, spending guidelines, and connecting you to relevant services and providers. We also help with developing budgets and overseeing your spending to make sure that it's aligned with your NDIS goals and objectives.

Designing Support Systems

We help design support systems for participants, taking on a participant-first approach that caters to the individual. We also assist with any conflict resolution that may arise between participants and providers.

Build Resilience of Supporters

We help to build the capacity of formal supporters (support workers), but also informal supporters (friends and family) to help them support the participant.

Coaching

We not only provide coaching to our participants of just the NDIS, but also important organisations that you might have to interact with in the future. These organisations include Centrelink, as well as medical and legal organisations to further build the capacity of participants in directing their own lives.

Refine and Reflect

Support Your Way consistently reviews its Support Coordination services with its participants - making changes and adjustments when necessary. We believe in reflecting and refining so that the coordination experience is fully optimised for the participant.

Crisis Management

We help with resolving any crises or any other problems that might occur. This might include developing strategies to prevent, mitigate and solve future crises.

NDIA Reporting

An important part of support coordination - we provide the NDIA with all the necessary information about our participants so that you receive the appropriate funding to meet your needs. We help to provide the participant’s narrative to the NDIA to ensure that the participant has a voice, as well as helping prepare any supporting evidence that is needed (for example, letters from therapists and other professionals).

In addition to our general Support Coordination services, we also provide these services to cater to those with specific needs:

Housing

Support Your Way have had multiple successful SIL (Supported Independent Living) and SDA (Specialist Disability Accommodation) applications, having had a wealth of experience in both fields. We also have experience with home modifications.

Assistive Technology

We support participants in gathering appropriate evidence - such as reports, quotes, assessments and applications - so that they can apply for assistive technology. In the past, we've dealt with applications regarding power wheelchairs, power beds and vehicle modifications.

Finding and Connecting To Quality Service Providers

We help in facilitating relationships between you and quality service providers whenever needed.

Large Scale Coordination

We have experience in coordinating within more complex environments, such as where many stakeholders are involved (for example - family members, support providers, nursing staff, Allied Health team and more).

NDIS Law

Through years of experience dealing with the NDIS, our team has a strong knowledge base to help you with any questions that may arise surrounding the NDIS and NDIS-related legislation.

Wide-ranging With All Types of Disability

We assist participants with all types of disability - having helped clients in the past with disabilities involving mobility, cognition, behaviour, learning, mental health, speech, swallowing and more. Our experience is wide-ranging, and we’re always able to help, no matter the disability.
