Report an incident

If you, a participant, a worker or a member of the public has been harmed — or could have been — we want to know. Reports are taken seriously, treated confidentially and acknowledged within one business day.

If anyone is in immediate danger, call 000 first.
Then let us know — we'll coordinate next steps with you, your coordinator and the relevant authorities.

Use our feedback form and tick "Incident". Anonymous reports accepted.

What happens next

  1. We acknowledge your report within one business day.
  2. A senior team member contacts you to understand what happened and what you need.
  3. If it's a reportable incident under the NDIS Commission, we notify them within required timeframes.
  4. We investigate, document outcomes and share findings back with you.
  5. We update training, rosters or policies so the same thing doesn't happen again.
